You intone a profane chant and weave necrotic sigils over a 1-ounce portion of flesh from the corpse of a humanoid dead for no longer than 1 week. As you consume the flesh, your body writhes and reshapes, taking on the exact appearance of the creature as it appeared in life, including its clothing and equipment. For the duration, you can switch between your true form and the assumed likeness as an action.
While in the assumed form, you can alter your physical structure to include or exclude one or two limbs (such as arms, legs, or vestigial appendages). Your weight can shift by up to half your normal weight. If the assumed form has wings, you gain a flying speed of 30 feet with poor maneuverability (as described in the Monster Manual). If it has gills, you can breathe underwater. You cannot assume the form of a creature whose size category differs from your own. You gain a +5 bonus to Charisma (Deception) checks to pass as the creature but do not gain its mechanical traits, such as ability scores, proficiencies, or special abilities.
If you take damage in the assumed form, you must succeed on a Constitution saving throw against your spell save DC (with a penalty equal to half the damage taken, rounded down) or revert to your true form. If you are reduced to 0 hit points, you automatically revert. When you cast this spell, you permanently lose one non-vital memory from your past (such as a childhood friend’s name or a minor life event), chosen by the DM.
When you revert to your true form for the first time after casting, or if the spell is dispelled, the necrotic energy recoils. You take 5d6 necrotic damage, or half as much on a successful Constitution saving throw against your spell save DC.
